I declare that I have been informed that I have the right to:
- to inspect the personal data being processed at any time,

- to object to processing and to transfer data,
- to withdraw consent at any time without affecting the lawfulness of processing, which was carried out on basis of consent before its withdrawal,
- to submit a complaint to the supervisory body.
